Fees: 

Initial Intake: $165

Individual Session: $155

Couple/Family Session: $200

I currently accept the following insurance providers: Premera Blue Cross Blue Shield, Regence, Providence, United Health, First Choice Health, Aetna, Asuris, Optum and Cigna. If I am out of your insurance network, I do offer super bills that you can submit to your provider for reimbursement based on your policy. Feel free to reach out with any questions.

Policies: 

Fees are due at the beginning or end of each session by debit/credit card unless prior arrangements have been made.  

My services are covered by certain insurance companies and I can bill upon your request if I am within your network. At your request I can also give you a monthly statement that can be used to bill your insurance company for out of network reimbursement (if it is within your insurance policy). In all cases, in order to get reimbursed from your insurance company I will need to diagnosis you with a mental health diagnosis that may become permanent on your medical records.      

*Cancellation/No Show Fees: 
In respect for our valuable therapeutic time together, I do ask for a minimum of 48 hours advance notice if you need to cancel or reschedule your session. Otherwise, you will be charged in full for the session missed. If there is an emergency (i.e. sickness, weather, etc.) that prevents you from attending your session, you will not be charged for the missed session if we are able to find a time within the same week to reschedule.
